Call (608) 829-0101 or send a request, and the office will find a time with you.

Fill out your forms
Download and print the forms at home. Email them in, or bring them with you to your appointment.

Meet the team
Your first appointment generally includes a screening, a review of your medical history, and an exam with a cleaning and X-rays.

We are in-network with Delta Dental. We accept out-of-network insurance as well.
